Care Home Visits
Bringing elite care to the residence of your loved ones.
Admission or permanent residence in care can be a difficult process to adapt to and is sometimes traumatic for all involved.
Why are care home visits needed?
It is imperative to maintain our mobility, strength, balance and cognitive function throughout our lives. Even when we may not be at our best or limited to a reduced quality of life there are still plenty of ways to maintain and improve these key properties. To be able to live independently or with aids in your own home, you need to be able to multi-task, function with minimal support and maximal safety.
We ensure a comprehensive review and evidence-based method to demonstrate and advise when it is appropriate for an individual to be discharged from care. In some cases, we may advise changes to care plans or even arrange further investigation for admission to residential care. Working with nurses, management and allied health professionals to ensure every physical aspect is covered to the highest degree.
